Scar is the biological process of healing and yet it is often considered unpleasant to the human eyes. Scars can occur on the scalp and face, or for that reason, anywhere on the body. However a small or large scar over the face and scalp can cause physical as well as psychological disability, in turn leading to loss of confidence in the person concerned.

Scars can be broadly divided for us into-

  1. Primary- Scars developed due to varying dermatological conditions including the Cicatrial/ Scaring Alopecias
  2. Secondary- This include accidental and non accidental traumatic scars as well as iatrogenic scars.

When the patient comes to a doctor for a transplant, he is quite pessimistic and is normally of the opinion that nothing much can be done. Scalp reduction surgeries has been implicated in these cases since long with varied benefits. Hair transplantation in  the hands of a good and experienced surgeon can produce amazing results and definitely looked into.

There exists few fundamental points to be kept in mind by a surgeon while working on such cases-

  • Nature of scar- Any type of scar is often in correspondence to ‘an area of decreased vascularity’. Adequate vascularity is paramount for successful outcome of procedure. A good blood supply can be evident from the looks as well as applying temporary pressure and noting the capillary refill and if blanching persists on release, a good result should not be expected, especially in certain thermal and post-radiation burns.  The surgeon should not attempt to achieve the usual density of 40-45 grafts per sq cm and aim much lesser density depending on scar tissue.
  • The thickness of scar varies even in a small area. This requires caution while creating recipient site ensuring the right depth and size. Scar tissue also tends to ooze more compared to normal tissues. Care is to be taken to not add insult to an already compromised tissue. Sometimes there is an indentation at the scar site due to fat atrophy and skin may be adherent to the skull in which cases micro fat transfer has been attempted by certain surgeons. In many cases where the scar is large and the skin loose and stretchable, excision of a portion of the scar tissue to reduce its size before undertaking the hair transplant is of great help to the surgeon.
  • Origin of scar – Primary scars due to inflammatory disease processes such as Lichen Plano Pilaris, Frontal Fibrosing Alopecia, Psedopalade of Brocq, Recurrent and severe folliculitis demand a more distinctive approach. Detailed clinical examination and scalp biopsy help determine the current disease activity status. A minimum of 4-5 years of ‘burnt out’ period is essential to devise transplant protocol here. Test grafting of 100 grafts with lower density in a particular area can be done if the absence of disease activity is not definite. When the graft take-up is good and hair follicles grow out over a course of one year; the surgeon can plan and optimise the full transplant accordingly.
  • Secondary scars due to accidental or non-accidental trauma and iatrogenic causes can be much simpler in contrast to its brethren. Since scar remodeling peaks at 3-6 months, a duration of one year post trauma is advised for completion of remodeling process thus ensuring better graft survival.
  • Lastly and yet importantly like any other hair transplant surgery a good donor area is necessary. The graft extracted are implanted by Follicular Unit Excision (FUE) / Direct Hair Transplantation ( DHT) technique with utmost care whilst graft handling, strictly following ‘ no root touch’.

Hair transplant done strategically bears fruitful results for the patient. He/She can use the hair to camouflage certain areas and live a more carefree life.

It is important to note that it is essential to educate the patient of the wavelength of the possible outcomes as scar tissue behaves differently from normal tissue.
